The Mattie Foundation was founded to assist those employed in the hospitality industry in Arizona with the reimbursement of expenses for the treatment of an illness or medical condition provided by a licensed healthcare provider. 

To be eligible to apply for financial assistance from The Mattie Foundation you must have verifiable employment in the Arizona hospitality industry.

Applicants are required to fill out an application for assistance and provide verifiable receipts for doctor's visits, prescriptions and/or lab fees to be considered for reimbursement.  Financial assistance grants up to $500 per person per year will be considered by The Mattie Foundation Financial Assistance Committee (FAC) which meets monthly to review requests for assistance.

*Expenses without receipts will not be considered eligible for reimbursement.
